Subject: - Request to cut a Dead tree

Dear Sir,

This is to inform you that one mango tree planted in our lawn has become dead. There are no leaves in this tree. The condition of this tree is such that it can fall at any time, which can cause damage to the life or the property. We would also like to inform you that last month ago such tree outside our lawn fell on the parked cars inside our premise during the heavy rains and damaged two cars badly. 

The tree inside our lawn is at the verge of uprooting and is likely to fell any moment during rains / storm. Therefore, we request you to depute your officer to inspect this tree in our premises request you to get it cut under your supervision.
