APPLICATION FORM (ANNEXURE A) 

The firm should read and fill-up the form carefully. The form should be signed and stamped. The scanned copy should be uploaded on the NET.

1. I / We am/are registered with the ……………………..as class …………….. (Category)…………………. Contractor/contractors eligible to tender up to Rs……………… It is further certified that the said registration is valid as on date.
2. I / we am/are not registered with DJB. We are manufacturer / authorized dealer etc. of the items……………………………..
3. It is certified that the firm has never been blacklisted or debarred form tendering by any of the govt. department or local body etc.
4. We are not having any relative in DJB or list of relative is (detail) …………
5. The firm undertakes that the documents (scanned copied etc.) if found false / fake, the department shall be at liberty to take action against the earring bidder.
6. The terms and conditions attached with BQQ will also be applicable.
7. I/We certify that we have gone through the General Conditions 1 to 45 Clause of contract S. No. 1 to 45, Instruction to bidders S. No. 1 to 11 DJB safety codes S. No. 1 to 16 Model rules for protection of Health & Sanitary arrangements for workers Employed in of by Delhi Jal Board or its contractor S. No. 1 to 12, Contractors Labour Regulations 1 to 18. We also agree to the content of tender document placed at Annexure B regarding the forfeiture of EMD / Performance Guarantee etc. We further Confirm of having full knowledge that the above conditions are to form a par to the contract agreement executed with the successful contractor.
8. The firm agrees to follow all rules and regulations as are applicable in DJB.
9. The decision of the Engineer-in-charge will be final regarding financial status/genuineness & eligibility of the firm.
10. The firm will also submit the above form duly filled –up at the time of submission of Bid
11. The firms will furnish an indemnity bond equal to approx. value of equipment & machinery.
