Subject: Application for obtaining information under Right to Information Act-2005.
I Abhihsek Jain, having Roll No: xxxxxxxx, a citizen of India, request you to kindly provide me following information Under RTI Act 2005 and details in connection to these missing/not traceable records of answersheets and OMR sheets for First year Annual Back Exam was held in the month of June-July in year 2010.
1) Please provide record retention schedule of the University.
2) Please provide copy of the policy for maintenance of the exam answersheets.
3) Certified Copy of a index of all records mainatained by the public authority in conformity with Sec 4(1)(a) of the RTI Act 2005.
4) Certified copy of a catalogue of all records mainatined by the university in conformity with sec4(1)(a) of the RTI Act 2005.
5) An index of all back paper answer sheets maintained by the University in comformity with sec4(1)(a) of the RTI Act 2005.
6) Cetified copy of a catalogue of all back papers answersheet maintained by the University in comformity with sec 4(1)(a) of RTI Act 2005.
7) Certified Copy of any documents/circulars/notifications/minutes of meetings which show the period for which answer sheets for back exams have to be maintained.
8) Please provide me certified photocopy of pages of registers where the records about First Year Annual Back Exam-2010 answersheets were last recorded to have existed.
9) Please provide me certified photocopy of pages of register where the records about my First Year Annual Back Exam-2010 answersheet for subject Mathematics-2 (paper code 103 ) was last recorded to have existed.
10) Name, designation and office address of the officers and other staff of the office who had last custody of the said records.
11) Date when your office first came to know that the said records are missing.
12) Details of efforts made to trace out the said records till date of your reply.
13) Certified copy of FIR filed for the missing/non-tracing of said records. In case if FIR is not filed till date of your reply to this application, reasons for it be also made known to me.
14) Certified photocopy of departmental guidelines for preservation and safe custody of records.
15) Certified photocopy of report made by your office to your higher officers and their reply on the matter of the said records.
16) Details of departmental action initiated against erring staff for the said records.
17) Certified photocopy of last audit queries and replies thereto in connection to said records.
18) Certified copy of record evidencing last periodical verification/inventory of old records undertaken by the dept/office as per prescribed procedure. If no inventory/verification is undertaken, reasons thereof be made known to me.
19) Please provide me a sworn affidavit to this effect in accordance with CIC decision dated 06-11-2009 in Appeal No.CIC/SM/A/2008/00135 & SM/A/2009/001075].
20) Has the University framed a policy/schedule for records maintainance, retention and destruction as per the Public Records Act 1993?
(i.) If yes, please provide a certified copy of documents that spell out the university’s official policy on records maintenance, retention and weeding out/destruction, including specific policy with respect to maintenance and weeding out of answer sheets.
(ii.) Has this schedule been vetted by the National Archives of India? When was the last time it was reviewed?
21) Name and designation of person nominated as records officer in RTU, as per the Public Records Act, 1993, responsible for maintenance, review and destruction of records.
22) What is the time schedule fixed by RTU for weeding out answer sheets as per you weeding out policy for the year 2010, 2011 and 2012?
23) Copy of the tender notices/advertisements taken out by RTU for the purpose of weeding out answer sheets for the years 2010, 2011 and 2012.
24) Outcome of the open bidding and copy of letter issued to winning bidders in each of the three years.
25) Please provide a copy of status report for implementation of Section 4 of the RTIAct, 2005, which clearly calls for suo-moto public disclosure of its entire working including rules, regulations, instructions, categories of documents held, minutes of meetings by its boards, councils or committees, details of employees and so on by every public authority.
26) Please provide a certified copy of rules/ordinances governing BTech.
27) Please provide a copy of ceritified affidavit which spell out that Rajasthan Technial University is following/obeying the Orinances(rules) of University of Rajasthan. (in accordance with CIC decision dated 06-11-2009 in Appeal No.CIC/SM/A/2008/00135 & SM/A/2009/001075)
28) Please provide a copy of certified affidavit which spell out that RTU is obeying and regarding the order passed by Hon’ble High court, Hon’ble supreme court and Hon’ble Rajasthan Information commission.
29) Certified photocopy of file noting and action taken report on this Application.
I am attaching the receipt of the payment of application fees for this RTI Application via Indian Postal Order Rs. 10/- only and serial no. 95E 167647. I am also attaching the Indian Postal order of Rs. (10*4=40/-) only containing Serial no. 95E 167704, 95E 167705, 95E 167706, 95E 167707 in advance towards the cost of Information(till 20 Pages) or return the remaining money if cost for information is low then advance paid Fee. I am willing to pay any further charges as decided by the PIO and communicated to me with a reasoned written order, giving detailed calculations for arriving at the additional charges so requested by the PIO.
If the information you are providing me is more than 25 pages please provide it to me in the form of a compact disc. If the information does not pertain to your office, please forward it to the correct office under section 6(3) of the Right to Information Act.

I need information regarding Pre. P.G. Entrance Examination for admission to P.G. Medical (MD/MS/Diploma) and Dental (MDS Courses) held on 11 & 14 feb, 2012
1. Give the list of students who appeared for (MD/MS/Diploma) examination as mentioned below
1.1 in service candidates of different category like SC,ST,OBC & Open
1.2 Non service candidates of different category like SC,ST,OBC & Open
2. Give the list of students who appeared for (MDS Courses) examination as mentioned below
2.1 in service candidates of different category like SC,ST,OBC & Open
2.2 Non service candidates of different category like SC,ST,OBC & Open
3. give me list of candidates according to their examination centre
4. Give me copy of documents which states methodology for allocation of examination centre.
5. Tell me whether examination for all the centre were conducted as per schedule or there was any problem in conduction of the examination at any centre.if the exams were not conducted as per schedule at any centre .then kindly give me the documents regarding the reasons and the report prepared.
6. Give me the copy of question papers which were used for the examinations.
